Contract of Care

The care of our patients is fundamental to the service we provide and it should be recognised that the patient has a responsibility in part for his or her own well being. We may prescribe treatments to aid a particular complaint or condition and may make recommendations for changes to and/or modifications in lifestyle to promote the patient’s well being.

Your responsibilities to us are to comply with any treatment recommended or prescribed, and to try and live as healthy a lifestyle as possible, asking for help and advice if necessary.

Data Protection

The practice is registered with the Information Commissioners Office (ICO) on the Data Protection Register as a data controller.

Confidentiality

All staff in the practice are bound to maintain patient confidentiality.  Any proven breach of confidentiality will be treated extremely seriously.

Confidentiality extends to family members, therefore medical information relating to you will not be divulged to a family member without your written consent (e.g. we cannot give information regarding appointment times or test results to your spouse or other family members without your prior written consent).

Access to Records

In accordance with the Data Protection Act 1998 and the Access to Health Records Act 1990, patients may request to see their medical records.  Such requests are by appointment only and may be subject to an administration charge.  No information will ever be released without your consent unless we are legally obliged to do so.

Data Sharing

How sharing information in your medical records can help the NHS to provide better care

A modern information system has been developed, which will make increased use of information from medical records with the intention of improving health services. The system is being delivered by the Health and Social Care Information Centre (HSCIC)  and  NHS England on behalf of the NHS.

The new system will provide joined-up information about the care received from all of the different parts of the health service, including hospitals and GP practices.

Your date of birth, full postcode,  NHS Number  and gender rather than your name will be used to link your records in a secure system, managed by the HSCIC. Once this information has been linked a new record will be created. This new record will not contain information that identifies you. The type of information shared, and how it is shared, is controlled by law and strict confidentiality rules.

If you are happy for your information to be used then you do not need to do anything. But if you have concerns you should talk to your GP.

If you do not want information that identifies you from being shared outside your GP practice, talk to a member of staff at your practice. They will make a note of this in your medical record. This will prevent your information being used other than where necessary by law, such as in case of a public health emergency. You will also be able to restrict the use of information held by other places you receive care from. However, this will not affect the care you receive.

For more information about how data is collected and shared, including confidentiality, please visit www.nhs.uk/NHSEngland/thenhs/records/healthrecords/Pages/care-data.aspx

Summary Care Record

There is a new Central NHS Computer System called the Summary Care Record (SCR). The Summary Care Record is meant to help emergency doctors and nurses help you when you contact them when the surgery is closed. Initially, it will contain just your medications and allergies.

Later on as the central NHS computer system develops, (known as the ‘Summary Care Record’ – SCR), other staff who work in the NHS will be able to access it along with information from hospitals, out of hours services, and specialists letters that may be added as well.

Your information will be extracted from practices such as ours and held on central NHS databases.

For Hucknall Road Medical Centre Patients it is assumed that you want your record uploaded to the Central NHS Computer System unless you actively opt out.

For further information visit the NHS Care Records Website.

If you choose to opt out of the scheme, then you will need to complete a form and bring it along to the surgery-please refer to the Records sharing form at the bottom of this page.

Compliments

Compliments are always welcome and are used to evaluate the services we offer.

Contacts

Contacts are not complaints but offer you a forum where you can raise any service or practice environment matter. Information provided will help us to improve the services we offer and the environment that these take place. Contact forms are available from reception. Alternatively use the contact us link above to post a contact.

Complaints

Complaints are always welcome and are used to improve the services we offer.

We hope that we can sort most problems out easily and quickly, often at the time they arise and with the person concerned. If you wish to make a complaint, please speak or write to the Operational Manager. For more information on the complaint process please read our  Complaints Leaflet

Complaints will be treated seriously and acknowledged in line with the Practice Complaints Policy and Procedure (available on request). Please do not use the contact us link above or the comments box (held in reception).

Chaperone

All patients are entitled to have a chaperone present for any consultation, examination or procedure where they consider one is required. The chaperone may be a family member or friend, but on occasions a formal chaperone may be preferred.

Patients are advised to ask for a chaperone if required, at the time of booking an appointment, if possible, so that arrangements can be made and the appointment is not delayed in any way. The Healthcare Professional may also require a chaperone to be present for certain consultations.

GP Net Earnings

All GP practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients at each practice.

The average pay for GPs working in Hucknall Road Medical Group in the last financial year was £69,770 before tax and National Insurance. This is for 2 full time GPs and 12 part time GPs who worked in the practice for more than six months.

Named GP

From 1st April 2015, GP Practices are required to provide a named accountable GP to all patients, rather than just the over 75. Your named GP will have overall responsibility for the care and support that our surgery provides to you. This does not prevent you from seeing any GP in the practice as you currently do.

If you are a new patient, you will receive a letter informing you of your Named GP. We will be informing existing patients of their named GP on ad-hoc basis on contact with the practice (the doctor listed on your prescription is your named GP) However, if you wish to have this information sooner please ask a member of staff when you are next at the surgery.

Patients over 75 will automatically continue to receive a letter informing of their named GP.
