Patients living within our practice area can request to join the practice. To register you will need to:
- Be in our catchment area
- Bring recent proof of your address (e.g. a utility bill from the last 2 months), and
- Complete the relevant forms at reception.
You will then be offered an appointment with a practice nurse for a health check. It is important that you bring any medication with you to your health check, this helps the practice to look after you until your full medical records are received from your previous doctor.
The Hucknall Road Medical Centre encompasses a close catchment area. If you wish to join the practice, or are an existing patient but moving address, please ask at reception to check if you are in our catchment area.

Once completed, bring in the registration forms with proof of your address to help us in registering you.
Local Practices
For further information regarding practices that cover your address or help in finding a practice to register with, you can contact NHS Patient Services (based at Byron Court) on 0115 883 1820.
If a practice, including ourselves, removes you from their list due to a change of address, NHS Patient Services will not normally contact you. Therefore it is your responsibility to ensure you and those residing at the same address are registered with a doctor's practice.
