One in three of us will get cancer and it’s the toughest thing most of us will ever face. If you’ve been diagnosed with cancer, or a loved one has, you’ll want a team of people in your corner supporting you every step of the way. Macmillan provide practical, medical and financial support and push for better cancer care.

Patient Registration

glasses_and_penPatients living within our practice area can request to join the practice.  To register you will need to:

  1. Be in our catchment area
  2. Bring recent proof of your address (e.g. a utility bill from the last 2 months), and
  3. Complete the relevant forms at reception. 

You will then be offered an appointment with a practice nurse for a health check.  It is important that you bring any medication with you to your health check, this helps the practice to look after you until your full medical records are received from your previous doctor.

 The Hucknall Road Medical Centre encompasses a close catchment area.  If you wish to join the practice, or are an existing patient but moving address, please ask at reception to check if you are in our catchment area.

Once completed, bring in the registration form/patient questionaire with proof of your address to help us in registering you.

Local Practices

For further information regarding practices that cover your address or help in finding a practice to register with, you can contact NHS Shared Business Services on 01162 957880.

If a practice, including ourselves, removes you from their list due to a change of address, NHS Shared Business Services will not normally contact you.  Therefore it is your responsibility to ensure you and those residing at the same address are registered with a doctor's practice.
